Skin Rejuvenation Frequently Asked Questions

Is everyone a candidate for the O’Neil Skin Rejuvenation?

No, of course not. But it is a rare person over 50 years of age who could not benefit substantially from this treatment. A consultation with Dr. O’Neil is required before scheduling. About 30% of the patients are screened out, at this time, for a variety of reasons including health, psychological make-up, or they may be best helped with a different procedure. Often the problem, the patient wants repaired, is better attacked surgically and Dr. O’Neil will refer you to a qualified Plastic Surgeon.

What type of health problems would preclude one from a skin rejuvenation treatment?

There is no definite answer here. It will depend on the severity of the illness, the patient’s age and other related health problems. For instance, he might exclude an unstable diabetic with a history of repeated infections but a stable diabetic, with no other health problems, would be fine for the treatment.

What type of tests are required?

All patients are first screened with their medical history by Dr. O’Neil. If he passes you at the consultation you are then sent to your own Doctor for a physical exam and blood tests. An electrocardiogram or chest x-ray may be required depending on your age and medical history. Information about the procedure will be provided to your Doctor and he or she must give their approval for the procedure.

What type of anesthesia is used?

An intravenous anesthetic using a sedative medication is used during the procedure. This will keep you asleep and unaware of discomfort during the procedure. This is less risky than general anesthesia and not as hard on your heart and lungs.

How long does the procedure take?

Dr O’Neil will be working on you for 1 to 1 1/2 hours.

What is O'Neil Skin Rejuvenation (OSR)?

O'Neil Skin Rejuvenation is a Proprietary treatment performed by Dr. O'Neil at the O'Neil Skin and Lipo Medical Center. It is non-surgical and is used to "tighten" and "lift" skin of the face, to remove wrinkles, to improve acne scars, and to improve sun damage problems such as large pores, pigmentation marks and rough skin.

How does it work?

O'Neil Skin Rejuvenation utilizes a chemical formula (peel), ultrasound, and a "cold" laser. These combined technologies cause a sloughing and re-growth of the surface skin. They also stimulate the deep tissues to grow new collagen. This tightens the skin, removing wrinkles, and improving elasticity.

How much younger will I look?

Results vary from patient to patient but most patients look 10-12 years younger following O'Neil Skin Rejuvenation.

How long do the results last?

Because the OSR tightens skin by regenerating new collagen fibers, it lasts much longer than chemical peels. An average patient in their mid 50's will look 10-15 years younger after the treatment and will remain looking 10-15 years younger than their true age for the rest of their lives.

What are the risks?

As with any medical procedure, usually your biggest risk is infection. We reduce infection risk by providing each patient with a prescription for antibiotic medication to take after the procedure and by using an antibiotic ointment and powder to the skin while it is healing.

What is the healing time?

After the initial 10-11 day stay, your beautiful new skin will be pink. Typically this will fade and disappear in 2 to 3 months.

Can the O'Neil Skin Rejuvenation be done on all skin types?

The OSR is safe and effective for all skin types including Asian or Hispanic skin.

Does it hurt?

Our patients are given intravenous anesthetic during the procedure to sedate them in order to minimize discomfort. After the procedure they move about freely while the healing continues. There is a sunburn like sensation for the first two to three days. Our patients are supplied with a few prescription medications to reduce itching or to help them sleep while they are recuperating.

How does the OSR compare to laser?

Compared to laser peels OSR offers greater skin tightening and age improvement. OSR removes deeper wrinkles and the results last much longer than laser peels. Also, the new skin is much smoother and more natural looking with O'Neil Skin Rejuvenation than with CO2 laser peels.

How does the OSR compare to a facelift?

Cosmetic Surgery on the face reduces redundant skin and pulls it tighter but it cannot remove wrinkles around the mouth and eyes where most of the aging occurs. And, of course, there is no improvement of the aging skin or complexion of the skin. The OSR removes wrinkles in all areas of the face. It gives the skin a younger, pretty look.

Will all of my acne scaring go away?

Dr. O'Neil will be very honest with you on what percentage of the acne scaring will be gone. You can discuss this with Dr. O'Neil at your consultation.

What is the difference between OSR and a Complexion Peel?

A complexion peel, which is also called a TCA peel, is a moderate strength peel that improves complexion on the face, neck and check. It can also be used on arms, hands or legs. It addresses age spots, fine lines, lesions, sun damage and brown spots. Typically this procedure is for the younger person, ages 25-40 or a previous OSR patient who needs a light maintenance peel. The complexion peel is not as effective at skin tightening or deep wrinkle removal as with the OSR procedure.





THERMAGE VS O'NEIL SKIN REJUVENATION

Thermage is a non-invasive skin treatment designed to tighten, contour and smooth out the skins surface. The Thermage machine, called Thermage Therma Cool NXT is a monopolar capacity radio frequency device. This machine or "tool" delivers high frequency energy into the skin and underlying tissues causing them to heat up.

The energy is aimed at the Dermis, heating this deeper layer of skin. The surface skin or the Epidermis is protected with a cooling spray. The heating of the Dermis causes new collagen formation and ultimately skin tightening. The results are gradual, improving for 2 to 6 months after the procedure.

It is commonly used on the face, tummy, arms, hands, thighs, and buttocks. Costs vary, depending on the physician, but usually run $2000 to $3000 per treatment area.

Thermage's claimed advantage is its non-invasive nature and lack of downtime, which is true. Results certainly vary depending on the experience of the physician.

O'Neil Skin Rejuvenation is a treatment protocol designed by Dr. Kelly O'Neil. The treatment is a combination of chemical peel and ultrasound therapy of the skin. He has used it successfully on approximately 9000 patients in the past 27+ years.

O'Neil Skin Rejuvenation (OSR) works by stimulating collagen growth in the Dermis causing tissue contraction, skin tightening and wrinkle removal. This is a result of dermal fibroblast (cells that form collagen) stimulation by the chemical formula and the heat generated by the ultrasound waves. The OSR treatment is designed specifically for the face and is not used on other body areas.

Thermage, by comparison, can be used on many body areas, was first approved for facial skin tightening. The skin tightening with OSR is more immediate than with Thermage. The skin tightening with OSR is 90% complete in 10 days versus 6 months with Thermage.

The formula used in OSR kills the surface skin cells stimulating re-growth of a new surface layer of skin. This removes and improves complexion problems such as actinic (sun) damage, acne scars, large pores, and other pigment abnormalities. Thermage has no effect on the skin surface hence no improvement on these types of problems.

The surface healing with OSR takes 10 full days and one should expect "down time" of 10 days to 2 weeks with OSR compared to none with Thermage. The new skin, with OSR, has an inflammation or pinkness to it that lasts several months and patients usually feel more comfortable with some make-up covering the first couple of months.

OSR requires some local anesthesia and intravenous sedation (twilight anesthesia) for patient comfort during the procedure. Some pain medication may be required the first few days, following the procedure, to dull a "sunburn" like feeling. Thermage is not painless but is usually performed without anesthesia. There is a mild to moderate burning sensation associated with the procedure and little if any discomfort after the procedure.

When comparing OSR and Thermage treatment of the face, there are two big differences in results. OSR offers surface skin improvements as well as skin tightening, Thermage offers only skin tightening.

The other difference is the amount of skin tightening and wrinkle improvement Thermage, at best removes fine lines around the mouth and eyes. Skin tightening is modest and in the range of 2 to 3 years of aging improvement, OSR, by contrast is capable of removing deep lines and wrinkles in all parts of the face. The skin tightening is much more dramatic and in the neighborhood of 10-12 years worth of tightening.

OSR is much more effective in stimulating new collagen growth in the Dermis and it is the new collagen that results in skin tightening and wrinkle removal. For the same reason OSR will last much longer than Thermage, 10-15 years versus 2 to 3 years with Thermage.

Thermage, at $2000 to $4000 is less costly than OSR ($5000 to $7000) but the added results and longevity of OSR will make up the cost difference in the long run.

In summary, Thermage of the face is best suited for younger patients who are looking for modest changes (fine lines and 2 to 3 years of aging improvement) and for those who have no time to spend "down" away from work while healing. For those patients who desire more dramatic results (10 to 15 years of aging improvement) and who have the time for a "down time" of 10 to 14 days, OSR would be the preferred treatment.

The results from O'Neil Skin rejuvenation vary dramatically from conventional skin peels. The quality of the new skin is better and more natural. These skin types are definitely more difficult to work with than fair Caucasian skin, and less experienced physicians are often afraid to work with them. O'Neil Skin Rejuvenation will alter the natural skin tone very little. While the tan is removed, leaving the skin slightly lighter than before, the natural color remains.

Most skin peels are designed to improve skin damage. Some even improve fin wrinkles. O'Neil Skin rejuvenation is designed to remove deep lines and wrinkles. It also tightens the skin in a way few skin peels do. This "tightening of the skin" results in a younger, fresher look, and leads to a longer lasting result. O'Neil Skin rejuvenation also includes mole and wart removal. Dr. O'Neil uses a high frequency electronic device, which is extremely sharp and literally shaves the mole from the skin. After peeling, the area where the mole was is often undetectable! Pores are cleaned and blackheads removed.

Precancerous skin lesions are also removed. Instead of leaving a scar, as with more conventional methods, there is just fresh skin. So if you are tired of visiting your dermatologist for burning and cutting sessions, this might be the treatment for you. O'Neil Skin rejuvenation not only removes the visible surface lesions but also the deeper invisible lesions that may be showing up next year.

Have you noticed the little broken capillaries in your skin? They are frequently found in women in the upper check area and by the sides of the nose. They are called telangiectasia and are a result of normal aging skin. They are aggravated by the sun exposure, alcohol, smoking, and physical trauma such as pinching a pimple. Conventional peels offer little improvement. They may even worsen them. O'Neil Skin rejuvenation will improve and reduce those fine blood vessels that are visible at the time of treatment. Future telangiectasia (fine blood vessels) treatments may be necessary, as the peel will not eliminate this forever.

The technique Dr. O'Neil uses in the skin rejuvenation program, have been refined over the past 27 year. They differ significantly from those used by other physicians. His formula, while using medically accepted chemicals, has been modified to produce superior results in skin tightening. It penetrates the skin better than other deep peel solutions while causing less burning of the surface skin. This results in a gentler peel but better wrinkle removal.

Some doctors have the patients use a preparatory program of creams and scrubs for seeks or months before the peel – others use nothing. The skin preparation is an extremely important part of any skin rejuvenation treatment, according to Dr. O'Neil. At Dr. O'Neil's center, the preparation of the skin is done immediately before the procedure, while the patient is asleep. It is extremely important to assure an even skin tone and good penetration of formula so the skin tightening is maximized. Most doctors spend little or no time on skin preparation before treatment. This is like painting an old house without cleaning the dirt and old paint of first. The result would be a poor quality paint job, uneven in areas and it would tend to chip after a short time. It wouldn't last! The skin preparation is just as important to ensure a good peel result. The skin contains fats and oils. The purpose of this is to keep things from penetrating the skin. They must be removed to obtain the proper penetration that leads to skin tightening and they must be removed completely to ensure even skin tone and color! This is why we spend so much time on this phase of our treatment at our center.

Another part of our preparatory treatment is microdermabrasion to the skin to remove the hard layer of surface skin. This allows for better penetration of our rejuvenation formula.

The peeling technique also differs. Most doctors, more or less, generally paint the solution onto the skin. Dr. O'Neil uses a formula that is milder than most deep skin peeling formulas. He rubs and scrubs it into the skin over a longer period of time. This leads to a milder peel, better skin tightening.

Finally the skin is treated with ultra sound to push formula into the dermis (deeper skin) where it can move effectively and produce collagen for tightening.

A good aftercare program is important to any procedure. Dr.O'Neil has designed a program, to make every effort for your comfort and healing to be maximized. The patient is recovered at the office by the nursing staff and then sent to the aftercare facility. The facility monitors them closely for the first 12 to 14 hours. After that, patients move about freely while healing continues. Patients are also issued other medication to reduce itching or help them sleep while at the retreat. There is minimal pain as compared to conventional peels.

O'NEIL SKIN REJUVENATION VERSUS FACIAL SURGERY

Facial surgery and O'Neil Skin rejuvenation are like apples and oranges. There is some overlap in function but, for the most part, they solve different cosmetic problems.

Facial surgery such as cheek and chin implantation, liposuction, or nose surgery are not applicable to this discussion. For our purposes, when I talk about cosmetic facial surgery, I am referring to face-lifting, brow-lifting and eyelid surgery. The function of this type of surgery is to remove excessive skin from the face. As we age our skin loses its elasticity (much like an old rubber band ). the loss of elasticity is caused by the degeneration of collagen and elastin fibers in the dermal (deep) skin layers. This leads to loosening of the skin and contributes to facial wrinkles.

Facial surgery removes most of the loose skin and there is obvious improvement in appearance. Wrinkles on the cheeks and forehead will be improved but not completely removed. Unfortunately, the surgeon most be more conservative with skin removal around the eyes, and little can be tightened around the mouth, otherwise distortion of these structures will occur negating the positive cosmetic results. This means that most of the wrinkling, in these areas, will remain.

Dermabrasion, an abrasive skin sanding technique, is often used around the mouth to remove wrinkles but not around the eyes. While it improves the mouth wrinkles, it usually leaves the mouth area hypopigmented (a lighter color).

Skin peels in general, and O'Neil Skin Rejuvenation particularly, are the most effective ways of removing facial wrinkles. Remember, loose skin and wrinkles are the result of collagen and elastin loss in the skin. Surgery does nothing to improve the collagen and elastic problem, it simply stretches the skin tighter. Deep skin peeling is the only effective method for regenerating collagen and elastin and most skin peels only minimally stimulate this regeneration so there is minimal skin tightening and wrinkle removal. O'Neil Skin Rejuvenation stimulates collagen and elastin regrowth like no other treatment. This attacks the loose skin and wrinkle problem at the source and results in distinct tightening of the skin of the face. The tightening is the result of contracting and shrinking of the skin, as if it were a place of cotton clothing after a hot wash. The "tightening" is even, proportional and very natural looking, as you can see in our patients.

O'Neil Skin Rejuvenation will give you a moderate amount of skin tightening, as compared to cosmetic surgery, but a much better improvement in facial wrinkles, especially around the eyes and mouth and, of course, other O'Neil Skin rejuvenation improvements are in skin tone, texture, pores, moles, cysts and skin capillaries (fine blood vessels) that are visible at the time of treatment, which are not improved by surgery at all. Future telangiectasial (fine blood vessels) treatments may be necessary, as the peel will not eliminate this forever.

O'Neil Skin rejuvenation is limited in the amount of skin tightening that can be obtained, and does not work well on the neck skin. Some patients may need some surgery in combination with O'Neil Skin Rejuvenation for optimal results. They are typically the older patients with redundant neck tissue and skin. In any case. O'Neil Skin Rejuvenation can reduce the amount of facial surgery required, saving you money and recuperation time. Most commonly, in these cases, O'Neil Skin Rejuvenation is combined with a "neck-lift" or lower face-lift procedures. The procedures cannot be done on the same day but O'Neil Skin Rejuvenation can follow surgery in 2-3 weeks or the surgery can follow O'Neil Skin Rejuvenation in 2-3 months.
